Hospice Nursing Assistant
The Hospice Nursing Assistant provides personal care under the supervision of a nurse according to the patient's plan of care.
This position will report to the Clinical Director of Hospice.
How to make an impact:
- Provide personal care and assist with ADL's according to the Plan of Care - POC.
- Provide safe environment for patients.
- Safely ambulate/transfer patient as appropriate.
- Help with ROM, both active and assistant active exercises as directed by the nurse.
- Prepare nutritional snacks as appropriate for the patient.
- Under the direction of the nurse, perform simple, skin care in accordance to the plan of care where appropriate.
- Recognize and promptly report any changes in the mental or physical status of the patient to the nurse.
- Record in clinical record, on appropriate documents, the activities provided and observations of patient at the time of visit.
- Complete all charting, timesheets and mileage logs as defined by the agency policy and procedures.
- Attend and actively contribute during patient care conference.
- Participate in Hospice discipline meetings, team meetings, staff meetings, retreats and in-services as appropriate.
- Participate and/or attend in-services training to obtain 12 education hours per year.
- Participate in the on-call rotation as scheduled
- Assist the Team Leader with other designated activities as necessary (scheduling, supply organization, answering office telephone, etc.)
- Demonstrate positive customer-service responsiveness according to agency standards.
- Flexible in meeting patient/family needs that require variable shifts and weekends.
- Participate in chart review quality improvement activities and other meetings as assigned.
- Participate in communication with volunteer staff.
What you’ll bring to the table:
- Experience in hospice, medical facilities, home care or nursing homes, preferred.
- Hospice/Palliative Care experience preferred.
- Must be state tested certified nursing assistant in good standing and have 12 months experience as a nursing assistant
- Knowledge of community resources.
- Comprehensive knowledge of state/federal regulations and industry standards
- Strong Administration skills as well as computer/software abilities
